Steps

  1. Identify your request

    • Are you asking about care/medications, insurance/billing, clinic directions/parking, or booking/rescheduling an appointment? If so your provider’s office is the right contact.

  2. Find your provider’s contact info

    • Check your after-visit summary or any welcome emails/texts from the clinic.

    • Look on the clinic’s website for the main phone number or patient portal.

    • If you received a message or video in Playback Health, use the clinic name shown there to search for their contact info.

  3. Prepare what to share

    • Patient full name and date of birth

    • Brief description of the request (e.g., “reschedule appointment,” “question about medication dose,” “insurance coverage for visit”)

    • Any relevant documents or images (attach when emailing their office or portal)

  4. Reach out to the clinic

    • Call the office or use the clinic’s patient portal to send a message.

    • Ask for the front desk or care team that handles your request type.

  5. (Optional) Use this quick message template

    • “Hello, I’m a patient of Dr. ____. My name is ____ (DOB: ____). I need help with ____ (e.g., rescheduling, medication question, insurance). Could you please assist or direct me to the right team? Thank you.”
