You can access your patient chart through Playback Health. Follow the steps below:
Step 1: Sign In
Go to patient.playbackhealth.com.
Sign in using your email address or phone number.
Verify the patient’s date of birth when prompted.
Step 2: Troubleshooting Common Errors
"Date of birth is invalid"
This error usually happens for one of these reasons:
If you’re a parent or caregiver, make sure you’re entering the patient’s date of birth, not your own.
Double-check that the date is entered in the correct format (MM/DD/YYYY).
